EverGreen Meadow Services Family Navigation and Therapeutic Support

Under the supervision of our Clinical Psychologist, a licensed therapist works directly with referred and eligible families to transition them therapeutically from hospital to home or the next level of treatment. This includes a family and adolescent needs assessment, short-term individual and family therapy, psychiatric support (as needed), and a plan for longer-term treatment. Adolescent girls in grades 6-8 may be referred to the EverGreen Meadow Services Let’s Tackle Trauma Together Program.

EverGreen Meadow Services clinicians reflect the diversity of New York families and are culturally responsive to the communities they serve. Services are available in English and Spanish.

Funding Opportunities

Hospital Community Needs Assessments that identify access to adolescent mental health services can serve their eligible population by referring at-risk patients and supporting funding needs to the EverGreen Meadow Services Hospital Discharge Program. To inquire, please get in touch with us.
